Fraser Health seeks an experienced part-time Team Leader to oversee and enhance our Home Health services. You will supervise staff, coordinate care plans, and deliver evidence-based practices. A Bachelor's degree in Nursing and supervisory experience in a community health setting is required for this position.
Key Responsibilities:
• Facilitate client care planning with the interdisciplinary team
• Oversee staffing and service flow
• Provide consultation and mentorship to staff
• Assess client care plans for feasibility and safety
• Lead educational initiatives for clinical staff
Requirements:
• Bachelor’s degree in Nursing
• At least five years of relevant experience
• Registered Nurse status with BCCNM
• Valid BC Driver's License for business use
• Strong problem-solving and communication abilities
