Become a Community Health Nurse at Fraser Health and support clients in enhancing their health. Work collaboratively in a community setting to promote wellbeing and care continuity.
In this role, you will serve as a Registered Nurse, working independently while participating in an interprofessional team to manage a caseload. You will focus on comprehensive client assessments, personalized care planning, and collaborative interventions that enhance health outcomes. Education and support for clients and families is crucial as you facilitate transitions across various healthcare services.
Key Responsibilities:
• Conduct individualized client assessments and interventions
• Create and coordinate effective care plans
• Provide health counseling and education to clients
• Collaborate with a range of healthcare professionals
• Support clients through transitions in service and care
Requirements:
• Registration as a Registered Nurse in British Columbia
• Experience working with complex geriatric and adult patients
• BC Driver's license for work-related travel
• Proficient in both verbal and written communication
• Ability to prioritize care for diverse client populations
