As a Licensed Practical Nurse within the Specialized Community Services Program, you will collaborate with an interprofessional team to deliver much-needed client services. Your expertise will help patients manage their health effectively and navigate the healthcare continuum, enhancing their quality of life.
Key Responsibilities:
• Build therapeutic relationships through effective communication
• Assess client needs and coordinate health services
• Develop individualized care plans in collaboration with teams
• Provide health counseling and education to clients
• Engage in quality improvement and risk management activities
Requirements:
• Licensed Practical Nurse registration from BCCNM required
• One year of clinical experience in community settings
• Valid BC Driver’s license and access to a vehicle
• Knowledge of community health resources essential
• Ability to work independently and as part of a team
