Become an Outreach Licensed Practical Nurse with Fraser Health and provide vital support to clients facing mental health challenges. Join a passionate team dedicated to patient-centered care in the community.
You will report to the Program Coordinator and work collaboratively with nurses to deliver holistic care. This role includes conducting assessments, administering medications, and helping clients with daily living activities. Your focus on respect and trust will enhance the recovery journey of those you serve.
Key Responsibilities:
• Administer medications per best practices
• Monitor patients’ condition and needs
• Participate in interdisciplinary care discussions
• Provide personal care in home and community settings
• Maintain accurate documentation for safety
Requirements:
• Graduate from a Practical Nursing program
• Current licensed status with BCCNM
• Valid certificates for First Aid, CPR, and Food Safety
• BC Driver's License and access to a vehicle
• Ability to build rapport and work independently
