Be a vital part of patient care as a Registered Nurse in Transitional Care at Fraser Health. This role highlights your ability to assess and implement patient-centered nursing interventions effectively.

Key Responsibilities:
• Assess and diagnose patient conditions accurately
• Collaborate with interprofessional teams for care planning
• Teach and guide patients and caregivers on health issues
• Document patient interactions and treatment plans diligently
• Involve in quality improvement and research activities
Requirements:
• Current RN registration with BCCNM
• Completion of relevant postgraduate nursing courses
• Certification in Basic Cardiac Life Support
• Strong ability in patient assessments and communication
• Commitment to person-centered and culturally safe practices
