Key Responsibilities:
• Implement care plans and monitor client progress
• Provide training and support to fellow team members
• Complete structured documentation and reporting
• conduct health assessments and medication administration
• Assist with clients’ daily tasks and personal care
Requirements:
• Current LPN License essential
• Compassionate and dedicated towards elder care
• Clear Criminal Record Check and Negative TB Test
• Excellent verbal and written communication skills
• Experience in community nursing or palliative care preferred
