We are seeking a compassionate and skilled Registered Nurse – Home Care Nursing to join a respected healthcare team serving communities across British Columbia.

In this role, you will provide comprehensive nursing care to patients within home and community settings, supporting a wide range of healthcare needs including chronic disease management, wound care, IV therapy, palliative care, and ongoing patient assessments. You will collaborate with physicians and interdisciplinary teams to ensure coordinated and evidence-based care delivery.

  • Conduct comprehensive patient assessments and ongoing evaluations within home care settings
  • Develop, implement, and monitor individualized care plans
  • Deliver skilled nursing care according to patient needs and treatment plans
  • Provide wound care management including advanced wound therapies and wound vac care
  • Administer IV therapy and monitor treatment effectiveness
  • Support palliative care patients and families with compassionate care and education
  • Collaborate with physicians and multidisciplinary healthcare teams
  • Provide education and support to patients and caregivers
  • Maintain accurate and timely clinical documentation using electronic systems
  • Foster safe, patient-centered, and supportive care environments

Join a Team That Makes a Difference
  • Active RN registration in good standing with BCCNM
  • Home care nursing experience preferred
  • Experience with CMOIS and Pixelere systems preferred
  • Trained in Home Care Nursing practices
  • Training and experience in Wound Care and Wound Vac management
  • Training and experience in IV Therapy
  • Training and experience in Palliative Care
  • Current CPR/BLS certification
  • Strong clinical assessment and care coordination skills
  • Excellent communication and teamwork skills
  • Preference given to candidates trained in CMOIS
  • Commitment to compassionate, evidence-based patient care
