The Community Health Nurse provides essential support to clients with complex health needs by conducting comprehensive assessments and care planning. Key Responsibilities:

• Screen referrals and assess clients’ health needs

• Collaborate with a multi-disciplinary care team

• Provide direct care and therapeutic interventions

Requirements:

• Current practicing registration as a Registered Nurse

• Minimum one year of clinical experience in a community setting

• Knowledge of health care services and community resources

• Access to a personal vehicle for work-related travel
