Impact the health of vulnerable communities as a Registered Nurse with Fraser Health in Urgent Primary Care. This role combines direct patient care with assessment, coaching, and advocacy in a collaborative environment.
You will provide effective nursing interventions within the Urgent Primary Care Centre, focusing on both scheduled and urgent needs. As part of an interdisciplinary team, you will build therapeutic relationships while promoting self-determination and connection to community resources. A trauma-informed approach is essential to ensure quality, culturally safe care.
Key Responsibilities:
• Conduct comprehensive assessments for clients seeking care
• Develop and monitor individualized care plans and goals
• Communicate effectively with clients and families for informed decision-making
• Advocate for client needs to optimize health outcomes
• Participate in outreach services to enhance care access
Requirements:
• Current RN registration with BCCNM
• Minimum one year related clinical experience
• Valid BC Drivers license and access to a vehicle
• Exceptional assessment and communication skills
• Ability to work in a fast-paced, changing environment
