Deliver community nursing care in Surrey as a Casual Registered Nurse with assessment and chronic disease management expertise. Work across clinic, home, and telephone settings.
Fraser Health is hiring a Casual Community Health Nurse - Registered Nurse for the Newton Wound Care Clinic. You will manage assigned client caseloads, support adults with complex chronic conditions, and coordinate care with interprofessional providers. The work includes client education, direct care, care planning, and transitions across primary and community services.
Key Responsibilities:- Assess clients and develop individualized interprofessional care plans
- Provide direct care, coaching, counselling, and health education
- Coordinate services with families, providers, clinics, and community resources
- Support care transitions, discharge, and referrals across healthcare settings
- Maintain clinical records, statistics, and progress notes
- Current Registered Nurse registration with BCCNM
- One year recent related clinical experience
- Experience assessing complex geriatric or adult chronic conditions
- Valid British Columbia driver's license and personal vehicle
- Ability to work independently and interprofessionally
