Regular Full Time
35 hours weekly
Working hours:
Early afternoons and mid-evenings (approx. noon to 8 pm)
There may be a need for earlier or later hours depending upon circumstances, including morning meetings and evening youth events, offering support at key times, and coordinating others to assist when not available.
Program
The Reconnect Program delivers street‑based outreach and short‑term, one‑to‑one support services to youth under 19 and identified as needing connection to community supports and resources. The program engages youth in community settings to build rapport, assess immediate needs, and provide timely, practical support. Reconnect focuses on stabilization and facilitates referrals and reconnection to appropriate longer‑term supports, including family, education, employment, housing, health, and other community‑based services.
Position Summary
Reconnect workers provide street outreach services, support and integrated case management to youth self-identified or referred by MCFD and other relevant community partners. Reconnect workers engage with youth who are initially identified as needing a connection to community support and resources to promote ways of engaging with longer-term support, family, community, employment, and other resources.
Responsibilities
Provide street outreach services to youth in the City of Surrey and other various neighborhoods as needed.
Establish relationships with youth to ensure they are connected to longer-term community supports.
Engage with and encourage youth to participate in pro‑social activities in the City of Surrey by offering access passes, recreation schedules, knowledge of community agencies offering youth‑specific activities and workshops.
Attend community meetings to identify youth and their specific needs, mapping and locating family and community networks, forming relationships with other service providers and agencies to ensure all information is current and relevant.
