Position: Outreach Support Specialist at Connective
Support vulnerable populations as an Outreach Support Specialist with Connective. This role focuses on personalized outreach to help clients integrate into their communities effectively.

Connective is hiring an Outreach Support Specialist who will facilitate clients' successful adjustment to community life. Ideal candidates will have experience in social services or in crisis management. Your responsibilities include supporting daily living challenges and devising strategies to enable client success, engaging with a myriad of professionals and family members for comprehensive care.

Key Responsibilities:

• Explore client needs through research and engagement

• Create and implement support plans with measurable goals

• Assist clients in navigating complex living situations

• Interact with families and service providers for better outcomes

• Address issues related to employment, education, and access

Requirements:

• One-year relevant experience in social services preferred

• Proven ability to assist vulnerable populations

• Must complete a Vulnerable Persons Criminal Records Check

• Valid BC driver’s license is mandatory

• Personal transportation required for client support
