During this 8-week role, you'll learn to create tailored support plans and gain experience in community inclusion through hands-on involvement. Key Responsibilities:

• Collaborate with clients on intervention and support plans

• Address client concerns such as housing and health

• Facilitate services to aid clients in achieving personal goals

• Work alongside professionals for optimal client outcomes

• Maintain support relationships with families and organizations

Requirements:

• Educational background in Social Services or Psychology

• Experience working with diverse populations

• Crisis response skills under pressure

• Valid driver’s license and personal vehicle

• Successful completion of a Vulnerable Persons Criminal Record Check
