Fraser Health is looking for Casual Social Workers to become part of the Long Term Care team at Carelife Fleetwood. This role involves conducting psycho-social assessments, providing clinical counseling, and facilitating interdisciplinary team conferences. Your expertise will help improve the quality of life for clients facing chronic illnesses and disabilities.

Key Responsibilities:

• Conduct psycho-social assessments of clients and families

• Provide clinical counseling for chronic illness adjustments

• Educate families on high-risk issues and adaptation strategies

• Coordinate initial admissions and orientations for new residents

• Advocate for client needs within interdisciplinary care plans

Requirements:

• Bachelor's Degree in Social Work from an accredited institution

• Minimum one year's relevant experience

• Current registration with the BC College of Social Workers

• Strong understanding of emotional health issues

• Ability to work collaboratively in a team environment
