Position: Social Worker for SHBBI Cases
Join the Fraser Health team as a Sexual Health and Blood Borne Infections Social Worker. Support clients living with HIV and HCV through comprehensive case management and educational outreach.

In this role, you’ll provide critical psychosocial support, crisis intervention, and manage intricate care plans for clients facing complex health challenges. You’ll connect with community agencies, enhancing client care and ensuring access to vital resources. Your collaboration with an interdisciplinary team will focus on improving health outcomes while fostering emotional wellbeing and stability.

Key Responsibilities:

• Deliver intensive case management to clients

• Address biopsychosocial difficulties through referrals

• Develop individualized care plans with the team

• Assess and respond to client needs continually

• Maintain thorough documentation and records

Requirements:

• Bachelor’s in Social Work required

• Minimum two years in healthcare settings

• Experience with high-risk populations

• Access to a personal vehicle

• Knowledge of relevant legislation and policies
