Part Time Community Care Social Worker

This role involves working within an interdisciplinary team to conduct comprehensive social work assessments and provide therapeutic counseling. You will engage with clients, develop care plans, and ensure efficient service delivery across the healthcare system. Additionally, your responsibilities will include liaising with community agencies, offering clinical support, and participating in research and training initiatives.

Key Responsibilities:

• Conduct social work and psychosocial assessments for clients

• Provide therapeutic services and facilitate client change

• Collaborate with teams for effective care planning

• Support patient admissions and discharges across services

• Share relevant information with community service providers

Requirements:

• Full registration with BC College of Social Workers

• Minimum 2 years in a complex health care setting

• Valid BC Driver's License may be required

• Ability to work independently and with diverse teams

• Comprehensive understanding of health care legislation
