The successful candidate will provide assessment and treatment services while collaborating with an interprofessional team. You will also play a key role in educating caregivers and conducting outreach work, ensuring continuous follow-up and support for clients. Your expertise will be crucial in promoting person-centered care and effective treatment plans.
Key Responsibilities:
• Conduct nursing assessments and develop treatment recommendations
• Provide educational sessions to community groups and agencies
• Support clients through home visits and crisis interventions
• Document patient care activities and progress accurately
• Engage in continuous professional development activities
Requirements:
• Current RN/RPN credentials with BCCNM
• At least two years of recent experience in related areas
• Access to a personal vehicle with a valid driver's license
• Strong assessment and crisis intervention skills
• Knowledge of treatment for mental health and substance use
