Part Time Primary Care Social Worker

Support community health as a Part Time Primary & Community Care Social Worker at Fraser Health's George Point Inn in Surrey, B.C. Engage with clients through comprehensive assessments and tailored therapeutic services.

This role requires a compassionate professional who will work with an interdisciplinary team to deliver impactful social work services. You'll conduct in-depth assessments, develop care plans, and coordinate with various healthcare providers to improve client outcomes. Collaborate with community resources and provide critical support to enhance the efficiency of service delivery.

Key Responsibilities:

• Facilitate comprehensive assessments for clients and families

• Implement therapeutic services to address psychosocial needs

• Collaborate in planning effective client care strategies

• Manage client transitions between acute care and community services

• Maintain thorough documentation and reporting of client progress

Requirements:

• Full registration with the BC College of Social Workers

• At least two years of experience in a health care setting

• Ability to operate independently and within a team

• Excellent communication and counseling skills required

• Valid BC Driver's License may be required
