Program Navigator: Mental Health & Housing Support
Phoenix Centre Mental Health and Substance Use Team has a casual Program Navigator opening on its Phoenix Society campus. You will support residents through recovery, discharge planning, and linking to housing and community resources.
Responsibilities include client services, crisis support, group delivery, documentation, and collaboration with partners to promote independent living in the community.
