Support inpatient psychiatry patients through comprehensive social work assessments, counselling, and discharge planning within Fraser Health’s integrated interdisciplinary team.
Fraser Health is hiring a Social Worker for inpatient psychiatry services. This position assesses acute mental illness and substance misuse, provides therapeutic counselling, and supports patients, families, and healthcare professionals. The work includes care coordination, community referrals, education, and consultation.
Key Responsibilities:- Conduct psychosocial assessments for patients and families
- Provide therapeutic counselling during acute mental illness episodes
- Coordinate discharge and community treatment plans
- Arrange referrals and connect patients with resources
- Document assessments, progress notes, and reports
- Master’s Degree of Social Work from an accredited school
- One year of recent related mental health experience
- Current registration with the BC College of Social Workers
- Current Class 5 Driver’s License and personal vehicle
- Knowledge of mental illness, addictions, and counselling
