We are seeking a skilled, compassionate, and experienced Qualified Practice Nurse to join the East Surrey Division of Modality Partnership on a part-time basis for 27 hours per week based at Townhill Medical Practice. This is far more than a traditional nursing role you will play a vital part in delivering high-quality, evidence-based care, preventative health services, and supporting innovation in primary care.

This role focuses on the delivery of high-quality, evidence-based nursing care to patients with chronic conditions, alongside the provision of preventative healthcare to the wider practice population. You will use your clinical expertise and professional judgement to assess, plan, implement, and evaluate patient care, demonstrating critical thinking, accountability, and excellence in every interaction. Working innovatively and flexibly, you will contribute to service improvement and support the effective delivery of care across the practice.
The role requires commitment, adaptability, and the ability to work resourcefully as part of a multidisciplinary team, ensuring safe, effective, and timely care inline with our CARE values.

- Clinical leadership skills with the ability to support and guide others.
- Clinical skills including cervical cytology, immunisation and vaccination, ear care, and minor surgery.
- Change-management skills with the ability to support patients in making lifestyle changes.
- Strong communication skills, both written and verbal.
- Ability to communicate sensitive or difficult information to patients and their families.
- Negotiation and conflict management skills.
- Ability to provide teaching, supervision, and mentorship in a clinical setting.
- Competent IT skills and confidence using digital systems.
- At least one year of experience in General Practice or community nursing.
- Experience in nurse-led management of chronic conditions.
- Experience implementing clinical guidelines and protocols.
- Experience in audit activity and quality improvement initiatives.
- Clinical supervision training and experience.
- Registered Nurse qualification with a minimum of two years post-registration experience.
- Knowledge of managing patients with long-term and chronic conditions.
- Understanding of accountability within your own role and the roles of others in Primary Care.
