We have an exciting opportunity for a Band 5 Nurse to work at Epsom PCN, within Surrey Downs Health & Care. If you are passionate about caring for people and making a difference within your community, we would love to hear from you!

We are looking for nursing staff from all backgrounds including newly qualified, those more experienced, looking for a change, maybe wanting to work locally, and excited by the difference of being out within the community with the benefits and challenges that brings.

To work within the team, contributing to team management and the development of innovative practice. To be responsible for the assessment of care needs and the development, implementation and evaluation of holistic care packages.

Please note these roles require post holders to hold a current and valid driving licence enabling them to drive in the UK and have access to a vehicle for work purposes.

We have our own Learning & Development Department that offers a range of different courses and development plans that give you the opportunity to grow your career.

• Work as part of the team undertaking lead responsibilities as negotiated and agreed with District Nurse.
• Treat all patients as individuals, respecting their privacy and dignity at all times.
• Assess individual care needs, implement and evaluate holistic programmes of care.
• Involving, supporting, informing and educating family/carers.
• Take every opportunity to promote health and wellbeing of the patient.
• Develop effective communication and liaison systems.
• Maintain clear and comprehensive, signed and contemporaneous records according to SDHC procedures.
• Facilitate/undertake training and professional updating for team and students.
• Act as an assessor and mentor to qualified and unqualified staff.
• Maintain, develop and monitor standards of evidence based nursing care.
• Maintain own professional and clinical integrity in line with NMC guidelines.
• Support/mentor team members and participate in clinical supervision activities.
• Work without direct supervision and maintain close liaison with the senior team member, deputising as required.
• Be responsible for maintaining timely patient care.
• Be responsible for data entry and other record keeping.
• Work in cooperation and partnership with all other professionals and agencies involved in the care of the patient.
• Be aware of and act upon, when necessary, procedures that are in place to protect vulnerable individuals.

• Registered Adult Nurse with NMC
• Valid Driving Licence (to drive within the UK)

• Understanding of current issues affecting the NHS
• Understanding of the Clinical Governance agenda
• Community experience

• Access to a vehicle for work purposes

• Ability to assess, deliver and evaluate quality care
