Nurse Practitioner/Physician Assistant – Azza Health Care Agency
We are seeking a Nurse Practitioner or Physician Assistant to join our interventional pain management team across multiple locations, including a physical therapy office, satellite clinic, and surgery center.

- Providing comprehensive assessments, diagnoses, and care plans
- Ordering, interpreting, and following up on diagnostic studies
- Documenting patient care in medical records

- Current applicable state NP/PA license and national certification
- DEA license or ability to obtain one
- Basic Life Support certification
This full-time position offers a salary range of $125,000–$150,000 annually.
Relocation to Sutherlin, OR is required before starting. Candidates should be prepared to work in person in a supportive, outpatient environment.
