Practice Nurse​/Diabetes Nurse

A practice nurse post with a diabetes focus, based across the Dordon and Polesworth surgeries of the Rural Warwickshire North Primary Care Network. The work centres on managing patients with Type 2 diabetes, including insulin initiation and complex long-term condition reviews, alongside treatment room services such as immunisations, wound care and cytology. The post suits nurses with diabetes experience or a firm wish to train towards Diabetes Specialist Nurse, working within a multidisciplinary team of GPs and advanced practitioners.

Current NMC registration as an adult nurse is required.

• Manage patients with Type 2 diabetes, including insulin initiation and complex long-term condition reviews
• Deliver treatment room services including immunisations, wound care, cytology and routine health checks
• Work alongside GPs, Advanced Nurse Practitioners, HCAs and administrative staff across both surgery sites
• Drive proactive, long-term health outcomes for the Dordon and Polesworth communities

• Existing diabetes management qualification or experience, or strong motivation to undertake diabetes training

• Current NMC registration as an adult registered nurse

• Solid clinical skills and adaptability
• Excellent communication skills

• Desire to work effectively as part of a multidisciplinary primary care team
