Adults Social Worker - Continuing Healthcare

Complete comprehensive social care assessments to support NHS Continuing Healthcare eligibility decisions. Contribute to multidisciplinary team meetings and provide professional social work recommendations regarding CHC funding. Assess mental capacity and apply the principles of the Mental Capacity Act when making best interest decisions. Advocate for individuals and families throughout the CHC assessment, review, and appeal processes. Work collaboratively with health professionals, providers, family members, and other agencies to coordinate care and support planning.

Undertake safeguarding enquiries and manage risks to ensure the safety and wellbeing of vulnerable adults. Support hospital discharges by facilitating timely assessments and arranging appropriate community care packages. Review existing care arrangements to ensure needs are met and funding remains appropriate. Maintain accurate case records, reports, and assessments in line with legislative and organisational requirements. Provide guidance and advice to individuals, families, and professionals regarding CHC processes, eligibility criteria, and available support services.

Manage a complex caseload, prioritising work effectively to meet statutory duties and timescales. Requirements of this role:
Must be a Qualified Social Worker, registered with Social Work England. Must have a Social Work qualification (Diploma in social work, social work degree or equivalent) Must have minimum of 3 years post qualifying experience working as a social worker
