General Practice Nurse

We are currently seeking an experienced Practice Nurse tojoin our dedicated team at Gresleydale Healthcare Centre. This opportunity hasarisen in response to the growth in our patient list, ensuring that our expanding patient community continues to receive high-quality care. The successful candidate will be fully supported by our established and welcoming nursing team and Nurse Partner.

This role offers an excellent opportunity to work within amulti-professional environment. You will collaborate closely with General Practitioners (GPs), Advanced Nurse Practitioners (ANPs), and Clinical Pharmacists, all of whom are committed to delivering outstanding patient care.

We are looking for a candidate with strong clinical skills,proficiency in IT, and excellent interpersonal abilities. The successful applicant should be enthusiastic about further developing their skills in response to the needs of the practice.

The post holder is responsible for the delivery of general treatment room nursing services to our growing practice population. Supported by our Lead nurse within the practice, they will deliver care within the boundaries of their role and undertake general treatment room services, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Key responsibilities

Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.

Implement and evaluate individual treatment plans for patients with a known long-term condition.

Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care.

Deliver opportunistic health promotion using opportunities such as new-patient medicals
