Manor Farm Medical Centre is looking to recruit an enthusiastic, compassionate and committed Advanced Nurse Practitioner on a permanent basis for 3 - 4 days, Monday to Friday. We are looking for someone who is able to support the wider clinical team, triage and prioritise the needs of our patients. Thepost holder will commit to demonstrating critical thinking in the safe clinical decision-making process.
They will communicate and work collaboratively withthe general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
You will need to be a degree qualified Advanced Nurse Practitioner with relevant clinical skills and expertise in order to work autonomously to help provide ongoing clinical care and manage acute demand. You will be confident carrying out telephone triage, clinical consultations, providing assessment, diagnosis and treatment arranging specialist referrals when indicated. Prescribing qualification isNOT essential, but if had, prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
About usManor Farm Medical Centre is situated in a busy market town with approximately 7900 patients serviced by our main site and a branch surgery. We are a friendly dispensing practice with 3 GP partners, 2 salaried GPs a very busy team of ANPs, Practice Nurses, Nurse Associates and HCAs, Receptionists, Dispensers and Administrators. We are a forward thinking practice and endeavour to provide a comprehensive service, as well as being a teaching and research practice associated with the UEA and involved in the training of F2 doctors.
We use EMIS Web and hold a 'Good' CQC rating.

Job Summary
Thepost holder is an experienced caring, compassionate and committed nurse who,acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis,treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will committo demonstrating critical thinking in the safe clinical decision-making process.
They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policyand procedures.
Key Responsibilities
Clinical Practice
- Assess, diagnose, plan,implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
- Telephone Triage
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
- Assess, diagnosis, plan,implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
- Diagnose and manage both acuteand chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
- Prescribing qualification isNOT essential, but if had, prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
- Work with patients in order tosupport compliance with and adherence to prescribed treatments
Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions - Prioritise health problems and intervene appropriately to assist the patient in complex, urgent oremergencysituations, including initiation of effective emergency care
- Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care. Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation
- Assess, identify and refer patients presenting with mental health needs in accordance with the NSFfor Mental Health. Implement and participate in vaccination and immunisation programmes for both adults and children
- Advise, support and administer vaccinations for patients travelling abroad, where appropriate
- Promote and deliver evidence-based care for patients presenting with aural conditions
- Meet the needs of patients presenting for opportunistic wound care
- To undertake Home Visits to housebound patients as needed and Care Homes if required
Communication
