• Provide direct patient care to residents, including assisting with daily living activities, mobilizing, and transferring patients as needed
• Maintain accurate and detailed records of patient care and vital signs
• Observe and report changes in residents' conditions to nursing staff
• Collaborate with the interdisciplinary team to develop and implement individualized care plans
• Support residents with medication management and administration
• Participate in in-service training and continuing education to stay updated on best practices and industry developments
