Responsibilities:
- Provide direct patient care to residents, including assisting with daily living activities, mobilizing, and transferring patients as needed
- Maintain accurate and detailed records of patient care and vital signs
- Observe and report changes in residents' conditions to nursing staff
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Support residents with medication management and administration
