PSALT Health Care Assistant - Community Substance Misuse
We are looking for an organised and highly motivated person centred individual to support the delivery of community drug services within the Swansea region. Kaleidoscope PSALT is a GP with Specialist Interest led service for individuals resident in Swansea aged 18 years +. Opiate Substitution medication is dispensed via a community pharmacy model. Excellent communication between primary care, secondary care and pharmacy colleagues will be required.
The Prescription Clark will generate and co‑ordinate prescriptions as directed by clinical colleagues. They will ensure that service user wellbeing and safe clinical practice is at the heart of service delivery. They will assist in the overall administrative tasks of the service, and be a critical link in the engagement of service users and community health professionals across the region.
duties of the job
Undertake triage assessment and co‑ordinate pre‑treatment clinical checks.
Facilitate outreach and community engagement to assess, commence and monitor opiate substitute therapy (OST).
Provide advice, information, and guidance on the management of substance use to service users engaged with services.
Agree treatment plans with service users, and review them regularly. Undertake drug screening (urine, fingerprint and oral).
Ensure case notes and other statutory records are up to date and kept in accordance with Kaleidoscope policies and procedures / regional information sharing protocols.
Deliver harm reduction interventions, including HCV testing and naloxone training/administration where appropriate and in partnership with Local Health Board and Public Health Wales colleagues.
Liaise with community pharmacies to ensure prescriptions are in place and monitored appropriately. Participate in clinical audits and service reviews.
Support referrals/liaison with Community Mental Health Teams (CMHTs) and other community substance use treatment providers on entering and leaving Kaleidoscope services.
Actively encourage service user involvement and feedback and ensure views are considered in informing the development of the service.
Liaise with primary care and sexual health services.
Deliver targeted recovery orientated psychosocial support interventions.
Be responsible for undertaking and recording patient observations eg. BAC, BP, Weight, Temp, O2 sat. etc.
