Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for a Support Worker within the Community Living Team to work Full Time based in Guernsey.

The salary for this permanent Support Worker job is up to £39,865 per annum.

  • To support patients/service users in undertaking activities of daily living within care plans.
  • To contribute to the assessment and development of care plans under the direction of a Registered Health or Social Care Professional.
  • To undertake the role of health promoter and educator for patients/service users and their support networks.
  • To recognise changes in patients/service users presentation which require the intervention of others and understand when to refer to the Supervision Registered Health or Social Care Professional and other colleagues for reassessment of patient care.
  • To report and elevate any safety, safeguarding or risk concerns to the appropriate member of staff and ensure that action is taken.
  • To build a therapeutic relationship with patients/services users and their support network, communicating with them in a way that respects their views.
  • To use a wide range of communication skills that can be adapted to meet the needs of complex and challenging situations.
  • To demonstrate effective communication skills with all members of the Multi-Disciplinary Team.
  • To provide clear, concise, and accurate electronic and paper records of the care given in accordance with agreed policy and standards.
  • To advocate on behalf of patients and service users.
  • To organise own workload and tasks, demonstrating the ability to recognise changing priorities and adjusting plans accordingly.
  • To positively engage with service developments and contribute to changes to policies and procedures.
  • To contribute to effective team working through communicating with others in a constructive way offering support and assistance and making suggestions for the improvement of care delivery.
  • To attend and participate in team meetings.
  • To have an awareness of cost implications and assist in the management of resources within the service area e.g. equipment and supplies.
  • To demonstrate the ability to develop new knowledge and skills, applying this to practice and sharing with colleagues as appropriate.
  • To keep up to date with evidence-based practice and current trends.
Requirements of this Support Worker role:
  • Approved programme of development at Diploma Level (Level 5 HE) or equivalent e.g. Level 4 BTEC Professional Certificate in Positive Behavioural Support
  • Understanding of relevant legislation.
  • Registered or eligible for registration.
