Integrated Care Clinician (Nurse)

The post holder will undertake telephone triage assessments of patients efficiently, advising the most appropriate intervention in accordance with best clinical practice, Trust policy, protocols and guidelines.

Work autonomously, assessing and advising callers within agreed guidelines.

Provide telephone triage and assessment for a broad range of patients, from minor ailments to complex and urgent conditions, within a remote clinical decision‑making environment.

Work as part of a multi‑disciplinary team alongside GPs, pharmacists and other specialists.

Ensure safe and appropriate advice or referral, supporting patients through the system and engaging with urgent care GP’s and pharmacists.

• NMC Registered Nurse (adult or child), relevant clinical practice as a nurse in front line, acute or chronic care, or telephone based triage or HCPC Registered Paramedic with demonstrable post registration experience as a Paramedic in front line services
• Evidence of undertaking regular update training and continuous personal and professional development as required by the regulating body

• Able to demonstrate strong listening and communication skills

• Able to work to own initiative and make decisions
  
  Able to demonstrate sound clinical decision making capabilities
• Be able to demonstrate high levels of current clinical knowledge, computer literate, committed to personal development
• Committed to ongoing training and development including mandatory updates
• Ability to work under their own initiative without direct supervision
• Ability to speak Welsh

• Flexible approach to work – expect to work a rotating shift system including working nights and weekends (unsocial hours)
