North Wilts Border PCN is seeking an enthusiastic Care Coordinator with Clinical Skills to join our growing multi-disciplinary team. You will help deliver personalised, joined-up care for our patients within a collaborative GP practice network in Wiltshire.
You will coordinate access to services, information, and support, ensuring care is timely, efficient and patient-centred. The role involves working with GPs, nurses, paramedics, social prescribers and wellbeing colleagues.
