La Fondation Saphir, acteur médico-social du canton de Vaud, recrute pour EMS Contesse à Croy un(e) assistant(e) en soins et santé communautaire (F/H/D) en CDI à 60‑80%. Vous accompagnerez les personnes dans leur vie quotidienne, en évaluant leurs besoins physiques et sociaux.
Sous délégation infirmière, vous participerez aux projets d’accompagnement et encadrerez les stagiaires. Maîtrise du français et Carefolio exigées; formation continue possible.
