Job Summary & RequirementsThe Clinical Nurse Specialist (CNS) - Wound Care works across the organization in a specialty-based focus and functions in an expanded nursing role, demonstrating a high level of autonomy and expert skill, to formulate clinical decisions and appropriately manage wound care. You will be part of the Professional Practice team, providing nursing leadership in knowledge-based practice, the development of standards, processes and quality initiatives enhancing the care for patients.
Responsibilities- Provides leadership in advanced nursing practice through the collaborative management of an independent caseload of patients within the specialty patient population
- Initiates and/or coordinates the implementation of a comprehensive plan of care developed in collaboration with other interdisciplinary team members as necessary
- Evaluates and modifies the plan in response to changes in the patient’s status
- Develops, implements, and evaluates initiatives that improve patient care quality in wound care, in accordance with the strategic goals of the organization and patient and family centered care
- Participates and provides leadership in policy, procedure and program development and evaluation
- Develops and maintains standards of nursing practice and management of patient care through the creation of an environment conducive to growth and development of staff and excellence in nursing practice
- Interprets and fosters the philosophy, goals, and best practice standards
- Initiates evidence-based practice and promotes a culture of safety, quality improvement, and accountability related to skin integrity and wound prevention
- Provides formal and informal education to nursing staff, students, and interprofessional teams on wound assessment, management, and prevention strategies
- Develops and delivers education sessions, competency-based training, and resource materials related to wound care
- Provides advanced assessment, diagnosis, and management of complex wounds, including pressure injuries, diabetic foot ulcers, venous/arterial ulcers, surgical wounds, and ostomies (if applicable)
- Acts as a clinical consultant to nursing staff, interprofessional teams, physicians, and allied health professionals on complex wound care cases
- Delivers, coordinates, and participates in education and professional development activities/programs
- Acts as a resource and serves as a consultant to individuals and groups within the professional community
- Bachelor of science in Nursing required
- Master of Science in Nursing required
- Provincial registered body membership required
- Current registered member in good standing with the College of Nurses of Ontario required
- Current certification in Basic Cardiac Life Support (BCLS) required
- Current certification in Advanced Cardiac Life Support (ACLS) preferred
- Minimum of five years current RN clinical practice experience with demonstrated clinical excellence in area of expertise
- One-year current experience (within the last 2 years) in enterostomal/wound care advanced practice role
- Experience in leadership role with the ability to work effectively within an interdisciplinary team
- Demonstrated knowledge of planning, developing, and implementing educational initiatives for health care providers and community members
- Demonstrates the ability to perform relevant best practices, knowledge integration and skills for the management of complex cases in a variety of care settings
