As part of a dynamic expansion in our Primary Care Action Teams at Thames Valley Family Health Team, we are looking for passionate Nurse Practitioners. You will be responsible for delivering comprehensive care to your panel of patients, emphasizing health solutions and education. This role allows you to play a significant role in transforming primary care delivery.
Key Responsibilities:
• Provide essential primary health care to individuals and families
• Focus on patient education and disease prevention strategies
• Collaborate within an interprofessional healthcare team
• Monitor and follow up on critical lab results after hours
• Leverage your skills in a community healthcare setting
Requirements:
• Active Nurse Practitioner registration with CNO
• Minimum 2 years of Nurse Practitioner experience preferred
• Baccalaureate of Science in Nursing required
• Proven leadership and interpersonal skills
• Must have reliable transportation for site visits
