Pay Information
  • Base pay: $77,200 - $106,200 per year
  • Pay per visit/unit: $49.00 - $69.00
Working Hours

40 hours per week.

Key Responsibilities
  • Assess and screen patients’ daily living and work-related skills; develop therapeutic retraining programs with measurable objectives.
  • Administer and interpret diagnostic and prognostic tests of function to assist physicians in evaluating patient’s level of function.
  • Confer with physicians and other health‑care team members and participate in development/revision of the Plan of Care treatment.
  • Provide therapeutic treatment and instruction to patients per physician orders to improve or restore strength, coordination, range of motion, and function, or teach compensation measures.
  • Review and expand retraining programs consistently with patients’ behavioral goals and within the rehabilitation program guidelines.
  • Consult with vocational team members to develop and implement vocationally oriented plans consistent with patient needs and capabilities.
  • Report patient status and progress to physicians, rehabilitation staff, Clinical Manager, and family members.
  • Participate in care coordination and discharge planning activities and act as a resource to other health‑care personnel for patient needs.
  • Design community reintegration activities to assist the client in physical reconditioning and psychological adjustment, coordinating the plan with interdisciplinary team members.
  • Recommend or design special adaptive equipment to improve residence or work environments or enhance participation in the rehabilitation program and community.
  • Design and train staff and family members to carry out the retraining program, including dressing, feeding, grooming, and hygiene skills; participate in team and family educational meetings.
  • Coordinate, oversee, supervise, instruct, and evaluate Occupational Therapy Assistants and Home Health Aides in implementing occupational therapy services.
  • Accurately document patient care observations, interventions, and evaluations on the day services are rendered; ensure interim orders from physicians are accurately documented and implemented; submit evaluations, treatment plans, and discharge summaries to the supervisor and care‑management staff.
Required Experience & Skills
  • Degree from an accredited Occupational Therapy Program.
  • A minimum of six months of occupational therapy experience (preferred).
  • Home Health experience is a plus.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Good organizational and communication skills.
  • Valid driver’s license, auto insurance, and reliable transportation.
