Position: CFSP Care Extender (Peer Support) Swain/Graham/Cherokee Co.

Sign‑on Bonus: $1,500.

Location

Field role for Region 1 covering Swain, Graham, and Cherokee counties. You must reside in one of these areas.

Hours & Travel

General business hours, Monday through Friday.

Travel within the region is required. When not in the field, work is performed virtually from home.

Role Overview

The Care Extender (Peer Support Specialist‑FC) engages with community partners, provides education, and strengthens relationships to support individuals and families navigating the CFSP Foster Care program.

Primary Duties
  • Engage in outreach, engagement, and follow‑up with members, caregivers, and families to support care management efforts.
  • Support care managers as they advocate for foster parents and other caregivers, addressing their needs or challenges with agencies and systems.
  • Coordinate services and appointments, such as wellness reminders and arranging transportation.
  • Assist the care manager in assessing and addressing unmet health‑related needs.
Requirements
  • HS diploma or equivalent and minimum 2 years of related experience, or an equivalent combination of education and experience.
Preferred Qualifications
  • Lived experience relevant to navigating similar challenges.
  • Peer Support Certification preferred.
  • Excellent professionalism and communication skills with individuals, providers, and stakeholders.
  • Clear verbal and written communication abilities.
  • Ability to build respectful relationships with individuals across a wide range of disabilities and health conditions.
  • Strong collaboration skills and ability to follow guidance from Care Managers and the Care Management Department.
