Occupational Therapist (OT) – Skilled Nursing & Rehabilitation

Kingston Care Center of Sylvania is hiring a compassionate Occupational Therapist (OT) to join our skilled nursing and rehabilitation team. In this role, you’ll design and implement therapy programs that help residents regain independence, improve daily living skills, and enhance their overall quality of life in a supportive, resident-focused environment in Sylvania, Ohio.

• Assess resident abilities and create individualized therapy treatment plans
• Provide therapy services to improve self‑care, mobility, and functional independence
• Monitor progress, adjust treatment plans, and maintain detailed documentation
• Collaborate with physicians, nurses, physical therapists, and other members of the interdisciplinary team
• Educate residents and families on therapy goals, adaptive strategies, and safety practices
• Ensure all therapy services meet Ohio state regulations and Kingston’s quality standards

• Active Ohio Occupational Therapist (OT) license required
• Bachelor’s, Master’s, or Doctorate degree in Occupational Therapy from an accredited program
• Experience in skilled nursing, rehabilitation, or long‑term care preferred (new grads welcome)
• Strong clinical, communication, and organizational skills

• Ability to work independently and as part of a collaborative rehab team
