Speech Language Pathologist (SLP) – Skilled Nursing & Rehabilitation | Kingston Care Center of Sylvania, OH
Kingston Care Center of Sylvania is seeking a passionate Speech Language Pathologist (SLP) to join our skilled nursing and rehabilitation team. In this role, you’ll help residents enhance communication, improve swallowing, and strengthen cognitive skills in a supportive, resident-focused environment in Sylvania, Ohio.
Responsibilities – What You’ll Do
- Conduct assessments, diagnose communication and swallowing disorders, and create individualized treatment plans
- Provide therapy sessions for speech, language, voice, swallowing, and cognitive-communication challenges
- Document evaluations, progress notes, and outcomes accurately in the EHR system
- Collaborate with physicians, nurses, physical therapists, occupational therapists, and families to ensure coordinated care
- Educate residents, families, and staff on therapy goals, safe swallowing practices, and communication strategies
- Maintain compliance with Ohio regulations and Kingston’s standards of care
Qualifications – What You Bring
- Active Ohio Speech Language Pathologist (SLP) license required
- Master’s degree in Speech-Language Pathology from an accredited program
- Certificate of Clinical Competence (CCC-SLP) preferred; CFY candidates welcome
- Skilled nursing, long-term care, or rehab experience preferred (but new grads encouraged to apply)
- Strong communication, assessment, and clinical documentation skills
