Physical Therapist (PT) – Skilled Nursing & Rehabilitation | Kingston Care Center of Sylvania, OHFull-Time or Part-Time | On-Site | Ohio PT License Required
Kingston Care Center of Sylvania is seeking a licensed Physical Therapist (PT) to join our skilled nursing and rehabilitation team. In this role, you’ll help residents recover strength, mobility, and independence while working in a supportive, team-focused environment. Whether it’s guiding someone through post-surgery rehab or helping a long-term care resident maintain quality of life, your expertise will make a lasting difference right here in Sylvania, Ohio.
WhatYou’ll Do as a Physical Therapist
- Evaluate resident abilities and design personalized therapy plans
- Provide one-on-one therapy to improve mobility, balance, strength, and daily function
- Monitor and document progress, updating plans as needed to achieve resident goals
- Collaborate with physicians, nurses, and therapy colleagues for coordinated care
- Educate residents and families on therapy goals, safe movement, and continued exercises
- Ensure therapy delivery meets state regulations and facility standards
- Active Ohio Physical Therapist (PT) license required
- Graduate degree in Physical Therapy from an accredited program
- Experience in skilled nursing, rehabilitation, or long-term care preferred (new grads encouraged to apply)
- Strong clinical judgment, communication, and organizational skills
- Compassionate, resident-first approach to therapy
- Ability to work independently and as part of a team
