Discharge Planner - C2
Trinity Health is hiring a full-time Discharge Planner - C2 to collaborate with the Clinical Care Manager, physicians, and the broader health care team to manage the progression of patient care and ensure optimal communication across the continuum.
Primary Responsibilities- Actively engage patients and families during the discharge process and provide education on choice, cost, and scope of services.
- Facilitate referrals to home care agencies, rehabs, DME vendors, home infusion vendors, and home lab as directed by the Clinical Care Manager.
- Assist with prior approval/preauthorization from insurance for post acute care services and identify and remove barriers to care.
- Explain Medicare important notice, appeal rights, and 3‑day qualifying hospital stay requirements for Sub Acute Rehabilitation coverage services when appropriate.
- Facilitate patient transfers to other health care organizations in accordance with hospital policies and all applicable State and Federal guidelines, including Joint Commission standards.
- Document all information pertinent to the transitional plan of care and provide post‑hospital care providers with the discharge plan.
- Advocate for culturally competent care and appropriate literacy level; utilize the teach‑back process to ensure patients/families are informed about their discharge plan.
- Associate’s degree in a health‑related field.
- Three years of experience in a health‑related field.
- Working knowledge of discharge planning and community resources preferred.

PayPay Range: $22.25 - $32.30 per hour. Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
