Job Summary

This pharmacist will primarily provide care and oversight for the safe and efficient transitions of care among patients admitted and discharged from the hospital. They will work as part of a dedicated team of transitions of care pharmacists, in collaboration with other healthcare professionals. This position will collaborate closely with the health system’s outpatient pharmacy to help execute our meds to beds program, including coordination of medication orders, medication delivery, and patient counseling.

Additionally, the pharmacist will provide inpatient pharmacy services, especially during weekends and potential evening shifts.

Practicing within a progressive clinical pharmacy model at Upstate University Hospital, pharmacists participate in clinical patient monitoring, sterile product preparation, medication distribution, transitions of care management, and patient counseling. Opportunities for teaching and scholarship are encouraged, allowing pharmacists to practice at the top of their license.

Responsibilities include interpreting medication orders, preparing compounds, dispensing medications, and providing drug information to healthcare practitioners. Pharmacists will also perform duties across various practice locations, including inpatient, emergency, ambulatory care, and transitions of care settings. They are expected to participate in quality assurance programs and maintain pharmacy records according to law and policy.

Minimum Qualifications

  • B.S. in Pharmacy or Pharm.

    D.
  • NY State license and registration – or eligible.

Preferred Qualifications

  • Previous hospital experience preferred.
  • Experience with hospital-based transitions of care activities preferred.

Work Schedule

  • Rotating days
