Position SummaryForward facing, the RCC is in relationship with residents, caregivers, and volunteers leading by example by providing exceptional care to residents. This includes all stages of the resident progression: pre-admission screening, the admission physical, care, education and training of staff and volunteers, and working on nursing issues with hospice. Work with residents/family members, certified home health agencies, licensed hospice, social workers and discharge planners.

EssentialJob Duties and Responsibilities (Additional duties may be assigned)
- Establish relationships with residents and family members upon admission and throughout the admission process providing the highest degree of care and emotional support ensuring the mission, vision, and values of Francis House are followed in order for all to feel surrounded by unconditional love of God in a peaceful, serene environment in a homelike environment.
- Provide quality care and services to residents and their families ensuring that resident needs are addressed and they are treated with respect and dignity. The Resident Care Coordinator will act in a caregiver role as needed and provide hands‑on care, physical and emotional support while maintaining a safe and comfortable home‑like environment.
- Demonstrate a high standard of nursing practice related to end‑of‑life care and pain and symptom management through competency evaluations. Address resident needs on admission through rounding, caregiver and/or family and resident huddles, and safety briefings, and monitor daily for changes in condition that are reflected in the care plan. Maintain accurate and detailed healthcare records so that any problem, plan of care, action, or goal are accurately and clearly stated, and changes are reflected as they occur and update medical staff and family members as needed.
Maintain accuracy in and storage of charting, incident reports and audits. - Provide information to potential residents and family members regarding Francis House, including the referral pool and possible admission, offering beds when available. Update referral pool as needed including entering data into database.
- Meet with administrative and staff members to seek input, review concerns, share professional support and education, and optimize communication to enhance the quality of resident care and professional growth.
- Ensure safe levels for all shifts. Consistently monitor schedule and fill open shifts appropriately.
- Work with the director, volunteer manager, and house coordinator to create meaningful opportunities for volunteers
- Initiate resident log on admission and update as needed. Coordinates admissions and interdisciplinary rounds efficiently.
- Ensure durable medical supplies and medications are available. Create medication profile and pre‑pour medications according to Francis House standards upon admission and as needed including collaboration with Hospice to ensure medication profile appropriateness for pre‑poured medications under the direction of the RCC and administered by unlicensed care staff.
- The Resident Care Coordinator will participate in regular on‑call rotations as part of the role and must maintain readiness and availability during designated on‑call shifts.
- Must be available to work at least two (3) weekends annually.
