Home Infusion Staff Pharmacist – Syracuse, NY (Direct Hire)
We are seeking a Home Infusion Staff Pharmacist for a Monday–Thursday schedule in Syracuse, NY. This is an opportunity to join a home infusion team supporting complex IV therapies and patient care.
Term of Assignment: Direct Hire
Schedule: Monday – Thursday (8:00 AM – 5:00 PM)
Pay: Starting at $125,000 (annually)
System: Care Tend
Job Duties:
- Review infusion orders and patient therapies
- Participate in clinical rounds
- Perform sterile compounding and TPN review
- Maintain patient records and care plans
- Ensure USP and cleanroom compliance
- Train and support staff
- Drive operational improvements
- Evaluate and monitor drug therapy based on the overall needs and clinical conditions of home care patients
- Collaborate with physicians, nurses, and other healthcare professionals regarding patient therapy
Requirements:
- Current New York Pharmacist license in good standing
- Minimum of 2 years of recent acute care experience OR 1 year of recent home infusion therapy experience preferred
- Strong knowledge and experience with intravenous admixture services and intravenous therapy
- Ability to evaluate and monitor drug therapy in collaboration with the interdisciplinary care team
