We are seeking a licensed Psychiatric Mental Health Nurse Practitioner to provide ongoing psychiatric services within residential and post-acute care environments. This role is well-suited for a mission-driven clinician who is passionate about improving access to mental health care for an underserved adult and geriatric population
. The position offers a consistent schedule, meaningful patient relationships, and the ability to practice independently in a non-acute setting.

• Perform comprehensive psychiatric evaluations and deliver longitudinal follow-up care
• Oversee psychotropic medication management and monitor clinical outcomes
• Develop and refine individualized treatment plans aligned with patient needs
• Partner closely with nursing staff, facility leadership, and interdisciplinary teams
• Communicate clearly and compassionately with patients and involved family members
• Deliver psychiatric care using patient-centered and trauma-informed principles
• Maintain accurate, timely documentation in accordance with regulatory standards
• Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP)
• Strong clinical judgment, communication, and interpersonal skills
• Ability to function independently while contributing to a collaborative care model

• Experience working with geriatric patients or within long-term or skilled nursing settings
• Prior background as a psychiatric RN or NP in institutional or residential environments is a plus
