At Nascentia Health, our Clinical Supervisors play a vital role in ensuring the delivery of safe, compliant, and high-quality home health services. You’ll lead and mentor clinical staff, support interdisciplinary collaboration, and help shape clinical operations—all while keeping patients at the center of everything you do.

This full-time, Monday–Friday (8:00 AM–4:30 PM) position reports to our Syracuse, NY headquarters and includes participation in an administrative on-call rotation. A fleet vehicle program is available to support this clinical role.

What You’ll Do as a Clinical Supervisor

You’ll oversee the coordination and delivery of high-quality home health services while supporting, guiding, and developing clinical staff. Your leadership will directly impact patient outcomes, regulatory compliance, and team success.

Key Responsibilities Include:

Quality and Safety of Patient Care
  • Ensure timely, safe, and compliant delivery of home health services, providing direct care when needed
  • Facilitate effective interdisciplinary collaboration and communication among patients, families, and caregivers
  • Review medical records, orders, and documentation to ensure quality and safety of care
  • Serve as a clinical leader and role model for professional practice standards
  • Address complaints, grievances, and conflicts in collaboration with the Associate Director
  • Investigate and report suspected cases of client neglect per agency protocols and in consultation with the VP of Patient Services
  • Participate in the administrative on-call rotation as needed
Clinical Staff Supervision and Development
  • Maintain consistent, clear communication with clinical staff
  • Conduct joint field visits to observe, support, and guide patient care practices
  • Ensure compliance with agency performance standards and expectations
  • Provide ongoing training, coaching, and professional development opportunities
  • Evaluate staff performance and provide timely, constructive feedback
  • Support staff accountability in managing caseload and care coordination responsibilities
Resource and Compliance Management
  • Serve as a resource for agency policies, procedures, and best practices
  • Guide staff in accessing and utilizing appropriate community resources
  • Ensure compliance with New York State and federal home care regulations, including patient rights and privacy laws
Professional Growth and Team Collaboration
  • Lead, facilitate, and participate in team meetings to promote a positive work environment
  • Collaborate with internal leadership, interdisciplinary teams, and community partners
What We’re Looking For!
  • Current New York State Registered Nurse (RN) license in good standing
  • Bachelor’s degree in nursing or related field from a NYS Department of Health–approved program
  • Valid New York State driver’s license, current auto insurance, and reliable transportation
  • Minimum of two (2) years of clinical experience in a home health agency setting
  • Strong assessment, critical thinking, and independent decision-making skills
