Registered Nurse – Home Health travel assignment in Syracuse, New York. Begin on 11/02/2026 for a 14-week assignment with guaranteed 40 hours per week. • Assess home health and hospice patients in stable and emergent situations and document findings.
• Implement, develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.
• Gather data on physical, psychosocial, and environmental factors and incorporate it into the Plan of Care.
• Maintain up-to-date patient records across visits, contacts, problems, plans, actions, and goals.
• Educate patients and families regarding care, medications, home safety, and self-care practices; tailor teaching to age, condition, culture.
• Supervise and provide clinical directions to HHA/STNA/LVN to ensure quality and continuity of service.

• Current New York State license as a Registered Nurse;
  Required.
• Proof of current CPR certification;
  Required.
• 2+ years of home health nursing experience and 1–2 years in a clinical care setting;
  Required.
• OASIS experience;
  Required.
• Homecare Homebase software experience;
  Required.
• Valid driver’s license and reliable transportation;
  Required.
• ADN or Nursing Diploma required; BSN preferred.
• CHPN certification;
  Preferred.
