Position Summary:
This position is responsible for oversight of CFS/Re-entry department, which includes case management services to clients coping with alcohol and substance abuse disorders and those reentering the community from in‑patient treatment and/or correctional facilities. This position will collaborate with social service programs throughout the Tribe and community in order to develop comprehensive services, including crisis intervention, counseling, outreach and referral services for in‑patient treatment.
In addition, the Director is expected to regularly assess, evaluate and facilitate community resources that will be needed by the client in an effort to reduce relapse/recidivism.
- Develops and implements department goals, program enhancements, policies and procedures and forms.
- Represents department at community and local meetings as needed.
- Participants on multi-disciplinary teams to assist clients with receiving appropriate care and resources, as well as participate in meetings, trainings, treatment planning, and case staffing as required.
- Secures emergent hotel stays for clients prior to treatment bed dates.
- Provides oversight of intervention measures/ activities, outreach and referral services to clientele who are experiencing chronic barriers of poverty, chemical dependency, domestic violence, gang violence, “post‑traumatic stress syndrome”, lack of coping skills and education that are infecting family stability.
- Oversight of staff providing individual/family counseling for clients by performing ongoing staffing of case plans to ensure clients’ needs are being met. Makes recommendations for resources. May provide case management for complex cases.
- Provides oversight over individual pre/post release plans or discharge plans; provides required assessment, coordinates with service providers, determines community resources needed for clients and families.
- Provides services and intakes to inmates incarcerated in the tribal jail or other correctional facilities, or those at in‑patient treatment facilities who can be diverted into the re‑entry program or referred to other tribal social service programs.
- Refer clients to internal and external resources as needed and monitor activities and progress.
- Develop rapport with clients in order to facilitate clients’ movement toward self‑sufficiency by ensuring that clients remain consistent with their conditions of release, probation, etc.
- Collaboratively develop with stakeholders, including external agencies like the Washington State Department of Corrections, Pierce County Corrections Bureau, King County Department of Detention and other Counties, data tracking measurements and information sharing systems; and, implement the measurements and systems.
- Responsible for being the Tribal contact with Department of Corrections. Assists other departments with navigating system to assist incarcerated Tribal Members.
- Responsible for completing department reports, including annual operations plans, quarterly reports, etc.
- Coordinate peer‑to‑peer mentorships, small talking circles and life skills classes for clients with other social service programs.
