• Current Nursing License in the Practicing State
• Valid Drivers License
• Current CPR Certification
• Two years experience as an LPN in a clinical setting, preferably in a home health or hospice setting
• Ability to use electronic medical charting and time-keeping software

• Administer skilled nursing care to patients in their place of residence as prescribed by the physician.
• Assist in providing nursing services and identify patient needs.
• Participate in planning, coordinating, and implementing patient care with the interdisciplinary team.
• Communicate effectively with patients, families, and healthcare professionals.
• Assist patients and families in learning self-care techniques.
• Provide care utilizing infection control measures.
